About Community Services Coordination Network

The Community Services Coordination Network (CSCN) is a not-for-profit organization, funded by the Ministry of Children, Community and Social Services, and the Ministry of Health.

CSCN staff work with families to coordinate access to various services and supports for children and adolescents who have complex multiple needs. The agency is the single point of access to residential based children and youth mental health services that are funded by the Ministry of Health. CSCN performs this function across a five-county catchment area that includes: London-Middlesex, Elgin, Huron, Oxford and Perth Counties.

Developmental Services Ontario - South West Region is a division of the Community Services Coordination Network.

CSCN is the Passport agency in the South West portion of the Ministry of Children, Community and Social Services, West Region. As the Passport Agency, we work with recipients and families in the following counties: London-Middlesex, Elgin, Oxford, Huron, Perth, Chatham-Kent, Sarnia-Lambton, Windsor-Essex, Grey and Bruce.

If you have questions about the Passport Program, please leave us your contact information and a detailed message at 1-519-438-4783, extension 1 and one of our Passport Coordinators will contact you.
